Transition to CommCare Corporation


Dear residents and families,
In October the skilled nursing centers currently owned and operated by CCI, Camelot Brookside, Camelot Rehabilitation at Magnolia Park, Camelot of Broussard, Camelot of Broussard Assisted Living and Northridge will transition ownership to CommCare Corporation. We want to take this opportunity to introduce ourselves and inform you of a few things that will take place during that transition.

Who we are…
CommCare is a Louisiana-based non-profit that now operates 14 skilled nursing facilities in the state. Our mission is to be an innovative leader in the development and delivery of quality health care services, and to be committed to a learning environment that attracts and develops dedicated care givers. We are committed to ongoing dedication of resources to continually improve the quality of care by investing in employees and their education, committing to technology and upgrading our centers. We are committed to strengthening relationships within the community and building relationships through community outreach programs.

Our culture…
The resident experience is our primary focus. Our philosophy is person-centered care in which we honor the individual and their choices. It is about empowering residents, giving them more choices, and changing the culture in which residents, families and staff members interact.

Next steps…

As the new owners of these skilled nursing facilities, our first step is to introduce ourselves and ensure that we know your wishes, that we get your informed consent on decisions regarding your care or the care of your loved one and that you understand policies that we will be implementing. A member of our team will begin reaching out to you this week to make an appointment to accomplish these things.
